According to the HOG file timestamp, this level was created in March 1997.
There are no credits nor date in the MN2 file.
There is a text file attached, but the author's name is not stated.

This huge building has three floors, plus attic and cellar. It's intimidating at first sight but fun to play. The "Hostage BBQ" is also funny, just a bit sadistic maybe.
